Liberty Dialysis offers comprehensive medical plans including coverage for hospitalizations, office visits, lab and x-rays, emergency care, prescription drugs and mental health and chemical dependency treatment. Coverage of alternative care treatments, such as acupuncture, and chiropractic care is included in some of the plans.
Our dental plan covers 100% of the charges for preventative care and a significant portion of the expenses for other dental work. Orthodontia coverage is also an available benefit under our dental plan.
The vision insurance plan covers eye exams, lenses, frames and contacts. There is access to a nationwide network of private practice optometrists and ophthalmologists, as well as conveniently located retail chain providers.
Flexible Spending Accounts
Liberty Dialysis recognizes that dependent-care and health-care expenses can significantly contribute to the costs associated with working. Paying these costs with pre-tax dollars can reduce the employee’s actual out-of-pocket costs. Health-Care and Dependent-Care Flexible Spending Accounts allows employees to pay their portion of qualified expenses with tax-free earnings.
Liberty provides basic life insurance coverage for benefit eligible employees. You may purchase optional life insurance for yourself and your dependents at a reasonable cost. Accidental Death & Dismemberment insurance is also available as a voluntary benefit.
Disability coverage provides partial replacement of lost wages when an employee is unable to work due to an illness or injury.

Paid Time Off
Liberty recognizes that taking time off enables an employee to maintain a proper work/life balance. Our PTO program offers employees flexibility in using paid time off for vacation, holiday and personal time. Additionally, extended sick leave is available in the event of an employees illness.
Employee Assistance Program (EAP)
This program provides short-term counseling for stress related issues, emotional difficulties, critical incidents in the workplace and other personal concerns, including financial, legal and daily living (a resource on child care, elder care, school information and more).
Liberty provides the opportunity for our employees to further their education through this program. We will reimburse you for approved tuition, registration, required books and lab fees.
